Formulario para Libro de Reclamaciones en Perú helps site owners publish and manage a digital complaints book form from WordPress.

The plugin provides a configurable public form where customers can submit claims or complaints related to the Libro de Reclamaciones used in Peru. After a successful submission, the plugin can generate a PDF receipt, send email notifications, and let administrators manage each claim from the WordPress dashboard.

Installation

From the WordPress plugin directory

Go to Plugins > Add New in your WordPress dashboard.
Search for “Formulario para Libro de Reclamaciones en Peru”.
Click Install Now and then Activate.
Go to Reclamos > Configuracion and complete the company information.
Create or assign the form, confirmation, and public lookup pages.

Manual ZIP upload

Go to Plugins > Add New > Upload Plugin in your WordPress dashboard.
Select the plugin ZIP file and click Install Now.
Activate the plugin from the WordPress Plugins screen.
Go to Reclamos > Configuracion and complete the company information.
Create or assign the form, confirmation, and public lookup pages.

FAQ

Does the plugin delete claims when it is uninstalled?: No, not by default. The plugin includes a setting that allows administrators to delete plugin data during uninstall when that behavior is explicitly enabled.
Can I test email delivery on localhost?: Yes. The plugin includes a local log mode that stores generated emails as HTML files, so you can test submissions without PHP mail, SMTP, or external email services.
Can I customize claim statuses?: Yes. Administrators can create and edit statuses from the WordPress dashboard.
